Accident Insurance Effective the 1st of the month after start date
Critical Illness Insurance Effective the 1st of the month after start date

Hospital Insurance Effective the 1st of the month after start date
Leave of Absence While on a protected leave (FMLA or MPLA), all benefits and premiums remain the same.
If a non-protected unpaid leave is 30 days or less, all benefits remain. Medical insurance is billed at the cost of the full premium. Other benefits are billed at the normal premium.
If a non-protected unpaid leave is over 30 days, medical insurance remains and is billed at the cost of the full premium. Dental insurance is canceled on the last day of the month that the non-protected leave starts and may be continued through COBRA coverage. All other benefits are canceled on the last day of the protected leave. Once you return to work, you have 30 calendar days to sign up for benefits.
Life Insurance Effective the 1st of the month after your 30 day benefit enrollment period ends
40% Long Term Disability Effective date is your start date
60% Long Term Disability Effective date is the date you enroll in this benefit (if this is elected during open enrollment, the insurance company will approve or deny any increase in coverage)
