The following State application forms have been modified for use in Wellesley, and incorporate reference to Wellesley's Wetlands Protection Bylaw.
The downloadable application forms and other documents are in PDF format, which requires Acrobat Reader 6.0 or higher. Paper versions of the application forms may be obtained at the Town Hall.
